The HDPhA would appreciate your participation in the Chemotherapy Toxicity Management Study.
The Systemic Treatment Program at Cancer Care Ontario is conducting a study to understand the management of chemotherapy-related toxicities by community pharmacists and your educational and communication needs to enhance toxicity management in community practice. The information gathered will help to identify areas where provincial quality improvement initiatives as well as development of CE programs may be of benefit.
The tool used in the study is a 15 minute questionnaire that can be completed online via FluidSurveys (https://cco.fluidsurveys.com/s/pharmacist-toxicity-management-survey/) or by completing a pdf version and faxing or emailing it to Kathy Vu at 416.971.6888. Both the online and pdf surveys will be hosted on the Cancer Care Ontario Drug Formulary website (https://www.cancercare.on.ca/drugformulary/).
The study period will run from April 1, 2016 to June 30, 2016. Our goal is to reach as many community pharmacists as possible to complete this survey. Please feel free to forward to your colleagues in community practice.
